Policies

  • The initial exam/consultation fee of $87.00 to $366.00 is to be paid by the patient on the first visit regardless of insurance coverage, including dual coverage. When possible, we will verify your dental coverage prior to your first appointment.
  • A payment is expected at each visit if the balance has not been paid in full on the first appointment. The last payment is expected prior to completion of all treatment (i.e. prior to being seated). If your appointments need to be spaced further apart to make a payment at each visit, please let us know.
  • Once treatment has begun it should be completed within 1-3 weeks. Please do not go more than one month between appointments, to avoid recurrence of infections.
  • We expect you to keep your appointments as scheduled (i.e. be on time). Please adhere to your appointment time so that each person may receive individualized attention and treatment. Please call and confirm your appointment. Saturday appointments that are not confirmed are subject to be rescheduled without notice.
  • We require 48 hours notice on cancellations or reschedules, or a $50.00 fee will be assessed to your account. A fee of $230.00 will be assessed for surgery cancellations. PLEASE understand that the answering service cannot make appointments or accept cancellations.
